Reconnect is an early intervention program in Perth designed to support Aboriginal young people aged 12–18 who are homeless or at risk of homelessness.

Who is this program for

Young Aboriginal people aged 12-18 who are facing, or who are at risk of homelessness.

About the program

Reconnect provides early intervention case management and assertive outreach to young people  who are homeless or at risk of homelessness. This includes those affected by family breakdown, family violence, child abuse, or housing insecurity (e.g., overcrowding, poor quality housing).

We can help

  • Stabilise and improve their housing situation
  • Gain ID documents, like a photo ID or Medicare card
  • Navigate complex systems, like Department of Housing or Services Australia
  • Apply for private rentals
  • Reconnect with family or informal support networks
  • Reconnect with education, training, or employment 

Our Youth Workers proactively engage with schools and other services that young people regularly attend or access to identify those in need and intervene early.

This program includes

In addition to addressing housing needs, we provide case management to support young people with:
Addressing other drivers of homelessness through skill building and connections to specialists supports such as food banks, alcohol and other drug supports and/or youth justice supports
Re-engagement or strengthening engagement with education, training or employment. This includes connections to teaching aids, collaborating with schools to design alternative learning programs or assistance with enrolment in vocation/training courses
Family reconciliation by working with families to address conflict and support the young person’s return to home or to establish a working support system for independent living
Improving mental health by building resilience, a positive sense of self-worth and self-sufficiency

Reconnect is open to anyone living in south east metro Perth aged 12-18, who is homeless or at-risk of homelessness, and is an Aboriginal or Torres Strait Islander young person.
